Enrollment Information for Crownpoint Community School

Required Forms to be completed for enrollment:

  • Enrollment Application  
  • Emergency Contact / Check Out Authorization Form  
  • Student Internet Account Agreement  
  • Application for Medical Services  
  • Authorization to Furnish Information and Assignment of Benefit (AOB) Form
  • Acknowledgement of Receipt of IHS Notice of Privacy Practices Form
  • Consent Form (IHS-47)

Required Documents for NEW Students:

  • Certificate of Indian Blood (CIB)  
  • Birth Certificate  
  • Language Usage Survey  
  • Immunization Record – (New and returning students)  

Kindergarteners must be 5 years old by Dec. 31 of the current school year.

Any student with a known medical issue, including a food allergy, must disclose the information to the school with a doctor’s statement.  

If you are enrolling a child and you are not the biological parent, you will need to provide proof of guardianship by presenting one of the following:

  • Legal court documents
  • Caregiver's Affidavit
  • Power of Attorney
