Enrollment Information for Crownpoint Community School
Required Forms to be completed for enrollment:
- Enrollment Application
- Emergency Contact / Check Out Authorization Form
- Student Internet Account Agreement
- Application for Medical Services
- Authorization to Furnish Information and Assignment of Benefit (AOB) Form
- Acknowledgement of Receipt of IHS Notice of Privacy Practices Form
- Consent Form (IHS-47)
Required Documents for NEW Students:
- Certificate of Indian Blood (CIB)
- Birth Certificate
- Language Usage Survey
- Immunization Record – (New and returning students)
Kindergarteners must be 5 years old by Dec. 31 of the current school year.
Any student with a known medical issue, including a food allergy, must disclose the information to the school with a doctor’s statement.
If you are enrolling a child and you are not the biological parent, you will need to provide proof of guardianship by presenting one of the following:
- Legal court documents
- Caregiver's Affidavit
- Power of Attorney
